Who is eligible to enroll in Health Carousel’s benefits?

All full-time employees who maintain an average of 30 hours a week are eligible for benefits. Eligible dependents include:

  • Legally married spouse
  • Child dependents under the age of 26
  • Child dependents over the age of 26 that are physically or mentally dependent on their parents or legal guardians

When can I make elections for benefits?
  • New Hire: You will have 30 days from the day you start employment to make your new hire elections. Coverage is effective the first day of employment. Failure to enroll will be considered a waiver of benefits.
  • Open Enrollment: Health Carousel’s annual Open Enrollment occurs every year in the fall. Changes made during Open Enrollment will be effective November 1st.
  • Qualifying Event: You will have 30 days from the date of your qualifying life event to make any mid-year changes, such as: some text
    • Marriage
    • Birth/Adoption
    • Divorce
    • Change of employment status
    • Coverage is effective the date of the life event.

I just enrolled, why am I not active in UHC/MetLife?
  • Enrollments in ADP are approved and then sent to the carriers on a weekly file feed every Thursday morning. Enrollments made in ADP after the feed is processed will be sent to ADP on the following week’s file.some text
    • United Healthcare processes the file on Thursday and will have coverage active by Thursday afternoon.
    • MetLife takes 24-48 hours to process the files and will have coverage active on Friday or Monday, depending on processing time.  
  • Regardless of when enrollments are processed, coverage will always be effective on the date of the hire or the date of the qualifying life event.

Why are my deductions off?

Any missed deductions, whether from a late enrollment or weeks taken off within or between assignments, will be made up on the following paycheck(s)

How do I get Insurance Cards?
  • Medical
    • www.MyUHC.com
    • You can access your card online.
    • Physical cards will also be mailed to your home address.
  • Dental & Vision
    • www.MetLife.com/MyBenefits
    • You can access your card online.
    • Physical cards will not be issued and are not required.
    • Your provider just needs to know the coverage is through MetLife and the employee’s Social Security Number.

When do my benefits end?
  • If your assignment ends and you do not have a contract signed for an assignment that starts within 28 days of your last day worked, you will no longer be active. Your benefits will end on the Saturday of your last week worked.
  • If you have a break of more than 28 consecutive days within an assignment, whether scheduled or unscheduled, you will no longer be active and your benefits will end on the Saturday of your last week worked.
  • For child dependents who turn 26, coverage will end on the last day of their birth month.

How do I continue my coverage once my benefits end?
  • You will be able to continue your medical, dental, and/or vision through COBRA continuation coverage. Sheakley is our COBRA administrator and you will receive your notice and enrollment information from them within the first few weeks of  leaving the company.
  • You will get information directly from MetLife on how to convert employee plans to individual plans for any/all portable coverages in which you are enrolled.

ADP Retirement Services takes roughly a month to process terminations when employment ends. Once fully processed, you can use your ADP login at www.mykplan.com or you can call ADP at 800-695-7526 to request a distribution.
